Abstract:

This presentation follows on from the presentation entitled 'A case management model for prison mental health care' and uses a case example to illustrate the strength of the model by following the individual's journey from referral to release. It demonstrates utility of a multidisciplinary approach, referral to a non-government organisation, intensive preparation for release and continuity of care to community services with connectivity to relevant community supports.
